Online Personal Training Questionnaire

Confidence in your training program comes not only from knowledge that your trainer is skilled, but also from knowing that your personal strengths and weaknesses were taken into consideration during your program design.

How will your trainer do this? In addition to knowing the common responses to training modalities, your trainer will spend a lot of time—hours, usually—reviewing the answers to your personal four-page questionnaire while collecting his thoughts and creating your unique program.

While completing the questionnaire, your training goals will become more clear, and those goals will be further honed during your initial contact with your trainer. He'll help you fine-tune your desires and sort out mixed goals in order to design a clear path to progress.
